Narrow QRS Complexes in a Patient with Baseline Surgical RBBB: What is the Mechanism?

چکیده
A 26-year-old woman born with Tetralogy of Fallot initially underwent a left Blalock-Taussig shunt followed by a complete repair with a transannular patch at 2 years of age. She complained of non-sustained short episodes of palpitations. Her Holter monitor showed episodes of six beat run of non-sustained ventricular tachycardia. She was scheduled to undergo surgical pulmonary valve replacement. In view of her symptoms and Holter results she underwent an electrophysiology study to map any substrate of ventricular tachycardia for intra-operative ablation. Her baseline electrocardiogram demonstrated complete right bundle branch block pattern (RBBB) with QRS duration of 174 msec secondary to surgical interruption of the right bundle branch. During the electrophysiology study she was noted to have intermittent narrow QRS complexes (Figure 1).The QRS complexes had a duration of 98 msec with no change in the PR interval and AH interval. What is the mechanism of narrow QRS complexes in this patient with baseline right bundle branch block (RBBB) pattern?

Interesting response of narrow QRS tachycardia to premature atrial complex. What is the mechanism?
Fig.1 shows a premature atrial complex (PAC) introduced during a narrow QRS tachycardia. What is the response and what is the interpretation? The tachycardia is a regular RBBB tachycardia with cycle length of 440 ms and ventriculo-atrial interval of 30 ms. There is near simultaneous activation of atrium and ventricle during tachycardia. Presence of His deflection prior to the QRS makes ventricu...
متن کاملComplete right bundle branch block of unique behavior: what is the mechanism?
A 59-year-old hypertensive man presented to the emergency department with an episode of dizziness without syncope. A surface 12-lead electrocardiogram (ECG) showed sinus rhythm at 82 bpm with a 2:1 complete right bundle branch block (RBBB) pattern (Fig. 1). The patient presented a full spontaneous recovery and was subsequently discharged and referred for ambulatory diagnostic work-up. Echocardi...
متن کاملNarrow QRS Tachycardia with Alternate Wide QRS Beats: What is the Mechanism?
A 44-year old lady underwent electrophysiology study for recurrent palpitations and documented narrow QRS regular tachycardia. The baseline ECG showed subtle preexcitation that was easily manifest on atrial pacing. The retrograde atrial activation sequence during ventricular pacing was eccentric suggesting retrograde conduction over the accessory pathway. A regular narrow QRS tachycardia with c...
متن کاملElectrocardiographic features and prevalence of bilateral bundle-branch delay.
BACKGROUND Definitive diagnosis of bilateral bundle-branch delay/block may be made when catheter-induced right bundle-branch block (RBBB) develops in patients with baseline left bundle-branch (LBB) block. We hypothesized that a RBBB pattern with absent S waves in leads I and aVL will identify bilateral bundle-branch delay/block. METHODS AND RESULTS Fifty patients developing transient RBBB pat...
متن کاملVariations of QRS Morphology in Patients with Dilated Cardiomyopathy; Clinical and Prognostic Implications
The QRS represents the simultaneous activation of the right and left ventricles, although most of the QRS waveform is derived from the larger left ventricular musculature. Although normal QRS duration is <100 millisecond (ms), its duration and shape are quite variable from patient to patient in idiopathic dilated cardiomyopathy (IDCM). Prolongation of QRS occurs in 14% to 47% of heart failure (...
